29 CFR 1926.102(a)(1): Eye and face protective equipment was not used when machines or operations presented potential eye or face injury:    a) 25 Pearl Street, Metuchen, NJ 08840. Employee was observed operating a STIHL TS420 14 inch Concrete Cut-Off saw, cutting masonry block while not wearing any eye and or face protection exposing him to eye and face injuries from flying debris.       Condition occurred on or about 03/16/16.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:    a) 25 pearl Street, Metuchen, NJ 08840. Employees were observed placing and cementing concrete blocks while working on and from tubular welded scaffold when the scaffold had not been inspected by a Competent Person before the start of the shift in that there was no fall protection system in place exposing the employees to fall hazards in excess of 39 feet to the ground below.      Condition occurred on or about 03/16/16.

Two employees were observed off- loading concrete mix and blocks while standing on a tubular welded scaffold platform with no fall protection systems in place exposing them to fall hazards in excess of 39 feet to the ground below.      Condition occurred on or about 03/16/16.
