29 CFR 1926.502(d)(15)(ii): Anchorages used for attachment of personal fall arrest equipment were not installed, and used under the supervision of a qualified person: a) Northeast corner of sixth floor: On February 24, 2016 employees performing framing activities used a snap hook tied back to a rope lanyard for an anchorage point. These components of the fall protection system were not designed for this application. b) Northeast corner of sixth floor: On February 24, 2016 employees performing framing activities wore improperly adjusted and improperly installed rope grab fall protection systems. The rope grab mechanism was adjusted to the very end of the rope lanyard. The snap hook on the 3 foot lanyard was then tied back to the rope lanyard just above the rope grab mechanism. These components of the fall protection system were not designed for this application. Note) Certification and documentation ARE required for this item.

Employees did not take action to minimize or eliminate the hazardous condition. b) Jobsite: On February 24, 2016 as part of their personal fall protection system, employees performing framing activities on the sixth floor of a multi-family structure did not recognize and/or acknowledge the hazard of installing and using the rope lanyard and rope grab mechanism backwards. Employees did not take action to minimize or eliminate the hazardous condition. c) Jobsite: On February 24, 2016 employees performing framing activities on the sixth floor of a multi-family structure did not recognize and/or acknowledge the hazard of not maintaining the proper adjustment on the rope grab mechanism to minimize the potential for a fall arrest.
