29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket: a) Jobsite, east side of Peters Creek, Peters Creek Road, Jefferson Hills, PA 15025: Observed on or about March 15, 2016, an employee was standing on the midrail of the basket of a Genie S-65 brand aerial lift while drilling and removing rivets from a section of railway bridge near tower 5. The employee was not wearing a body belt with a lanyard attached to the boom or basket. The employee was exposed to a fall hazard of approximately 30 feet to the ground below.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a) Jobsite, east side of Peters Creek, Peters Creek Road, Jefferson Hills, PA 15025: Observed on or about March 15, 2016, employees working from the basket of a Genie S-65 brand aerial lift while drilling and removing rivets from a section of railway bridge near tower 5 were not wearing a body belt with a lanyard attached to the boom or basket. The employees were exposed to a fall hazard of approximately 30 feet to the ground.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Jobsite, east side of Peters Cree, Peters Creek Road, Jefferson Hills, PA 15025: Observed on or about March 15, 2016, multiple employees were renovating a portion of the steel framework that was located on the ground less than five feet from the edge of a wall with an unprotected edge. The employees were exposed to a fall hazard of approximately 9 feet 7 inches to the ground below. b) Jobsite, east side of Peters Creek Road, Jefferson Hills, PA 15025: Observed on or about March 15, 2016, a Champion brand portable generator (model number 46514) was located less than a foot from the edge of a wall with an unprotected edge. Employees accessing the portable generator were exposed to a fall hazard of approximately 6 feet 8 inches to the ground below.
