29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  The employer is failing to protect employees from struck-by hazards in the production areas.  This was most recently documented where employees operating the Hyster forklift to move materials throughout the production areas were not provided training to ensure each operator was competent to operate  the powered industrial truck safely.
