The violation read as follows:       29 CFR 1926.404(f)(6): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent and continuous:     a. On or before March 16, 2016, where employees were using an air compressor to provide compressed air to nail guns, the ground prong was missing from the plug of the air compressor, thereby exposing the employees to a 125 volt electrical contact hazard.     b. On or before March 16, 2016, employees were using an extension cord that was missing the ground prong to provide power to a circular saw, thereby exposing the employees to a 125 volt electrical contact hazard        b. In regards to citation 1, item 2, issued on March 29, 2016, the employer has failed to submit abatement certification within ten days of the abatement date, April 8, 2016.  The violation read as follows:       29 CFR 1926.405(a)(2)(ii)(J): Extension cord sets used with portable electric tools and appliances shall be of three-wire type and shall be designed for hard or extra-hard usage. Flexible cords used with temporary and portable lights shall be designed for hard or extra-hard usage.  NOTE: The National Electrical Code, ANSI/NFPA 70, in Article 400, Table 400-4, lists various types of flexible cords, some of which are noted as being designed for hard or extra-hard usage. Examples of these types of flexible cords include hard service cord (types S, ST, SO, STO) and junior hard service cord (types SJ, SJO, SJT, SJTO):     a. On or before March 16, 2016, employees were using an extension cord that was not designed for hard or extra-hard usage, thereby being exposed to a 125 volt electrical contact hazard.

The violation read as follows:      29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets:      a. On or before March 16, 2016, an employee without head protection, working below employees on the roof of the house who were installing bracing and sheeting to the roof, was exposed to struck by hazards.    Diamond Ishmael was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.100(a), which was contained in OSHA inspection number 314593245, citation number 1, item number 1 and was affirmed as a final order on July 26, 2011, with respect to a workplace located at 14900 Havens Corner Road, Pataskala, Ohio 43062.    b. In regards to citation 2, item 2, issued on March 29, 2016, the employer has failed to submit abatement certification and documentation within ten days of the abatement date, April 8, 2016.  The violation read as follows:                               29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities six feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502.  Note: There is a presumption that it is feasible and will not create a greater hazard to implement at least one of the above-listed fall protection systems. Accordingly, the employer has the burden of establishing that it is appropriate to implement a fall protection plan which complies with 1926.502(k) for a particular workplace situation, in lieu of implementing any of those systems:    a. On or before March 16, 2016, where employees were on the roof of the house installing bracing and sheeting to the roof, the employees were not protected from falls, thereby exposing the employees to a ten foot fall hazard.
