OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to struck by hazards from falling stock stored on unsecured racks.  On or about March 17, 2016, employees were exposed to struck by hazards due to falling product, such as printing materials, from unsecured storage racks throughout the warehouse.  The employer failed to ensure that all storage racks were securely anchored to the floor.  No abatement certification or documentation is required for this item.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  On or about March 17, 2016, employees were exposed to skin irritation and burns while using propane and the employer did not adequately train employees on the associated safety and health hazards of the chemical.  Abatement certification is required in accordance with the requirements of 29 CFR 1903.19(c).
