OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from a fall  a) Building Exterior/Roof Level: The employer does not ensure that personal protective equipment was used when necessary whenever hazards capable of causing injury and impairment were encountered: The employer did not ensure that a personal fall arrest system was in use to prevent an employee who was engaged in window washing work while standing on a 6 on 12 pitched roof from falling approximately 14 feet to a lower roof. Condition noted on or about 03/14/2016.
