29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On or about March 9, 2016:  a) Employees using pneumatic nail guns and saws to preform framing tasks, were exposed to eye injuries when the employer failed to ensure that the employees were wearing proper eye protection, to eliminate or reduce the hazard.

29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide.  On or about March 9, 2016:  a) An employee utilizing a job built scaffold was exposed to fall hazards of approximately 4 feet to the inside of the structure and approximately 15.9 feet to the outside of the structure when the employer failed to ensure that the scaffold platform was at least 18 inches in width.

On or about March 9, 2016:  a) An employee utilizing a job made scaffold was exposed to fall hazards of approximately 15.9 feet. When the employer failed to ensure that the employee was protected from falling outside the structure by installing fall protection means to protect the employee utilizing the scaffold system.
