29 CFR 1926.21(b)(2):  Workers were not instructed in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control  or eliminate any hazards or other exposure to illness or injury:    (a) At the patio, on or about March 23, 2016, sub-contractor workers who were removing the entire roof structure during a renovation project were not instructed in the hazards associated with their work, such as demolition operations, resulting in a struck-by hazard.

29 CFR 1926.850(a): An engineering survey was not performed by a competent person to determine the conditions of the framing floors and walls and the possibility of unplanned collapse of any portion of the structure prior to permitting workers to start demolition operations:    a) At the patio, on or about March 23, 2016, an engineering survey was not performed by a competent person prior to subcontractor workers  removing the roof from the patio, resulting in a struck-by hazard.
