The Lairds Lighthouse Inc dba The Lighthouse Restaurant - On or about 3/25/16, and at times prior thereto, caution signs were not affixed to the Hobart Meat Tenderizer and/or in the meat tenderizer area of the kitchen to caution personnel against the hazards of reaching into the machine and contacting the tenderizing wheel.    B.)  The Lairds Lighthouse Inc. dba The Lighthouse Restaurant - On or about 3/25/16, and at times prior thereto, caution signs were not affixed to the Hobart Meat Tenderizer and/or in the meat tenderizer area of the kitchen to caution personnel to unplug or lockout the tenderizer before reaching into it.

The Lairds Lighthouse Inc dba The Lighthouse Restaurant - On or about 3/25/16, and at times prior thereto, employees were bypassing a guard to reach into the Hobart Meat Tenderizer to clear jams. These jams included but were not limited to meat that was jamming inside the machine.  Employees were not unplugging/maintaining exclusive control of the plug while performing these activities or locking out the machine.
