29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6: a) Packaging Department/Bottle Line 1: A compressed gas cylinder in storage, such as but not limited to a Nitrogen compressed gas cylinder was not stored in accordance with Section 3.3.8 of the Compressed Gas Association Pamphlet P-1 1965. CSHO observed a Nitrogen cylinder that was free standing and unsecured by Bottle Line #1, on or about 06/10/2016.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by rotating parts: a) Prodo Pak # 3 and 4 Area: Employees were exposed to the an amputation hazard from the unguarded rollers of the Prodo Pak # 3 and 4 packaging machines, on or about 03/29/2016. b) Proto Pak # 1 Area: The guard at the rear of the Proto Pak # 1 packaging machine was missing. Employees were exposed to an amputation hazard of from the unguarded rollers of the Proto Pak # 1 machine, on or about 03/29/2016.
