29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l) and as referenced by the note contained in 1926.602(d):  a)  On or about March 28, 2016 at Lot 74 Crystella Drive in Menasha, employer failed to provide written operator training/evaluation certification for an employee operating a Caterpillar TL1055 rough terrain forklift.  The employee was lifting roofing materials up to the roof surface.
