29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) 1223 Bittner Blvd, Lebanon, PA; An employee was repairing and servicing a rooftop HVAC unit 15 feet 4 inches above the ground without the use of fall protection, on or about 03/30/2016. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) 1223 Bittner Blvd, Lebanon, PA; An employee used a Louisville 24 foot extension ladder that was not placed 3 feet above the landing to access the rooftop work area, on or about 03/30/2016. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
