29 CFR 1926.451(b)(7): On scaffolds where platforms are overlapped to create a long platform, the overlap did not occur only over supports:  (a) New townhome construction, west exterior wall: On or about 03/29/2016, and at times prior, the employer exposed employees to fall hazards of 21-feet, 3-inches from the ladder jack scaffold platform, in that the bridging of the scaffold platform did not occur over a support. The overlapped scaffolds occurred over a cantilevered section of the pump jack scaffold.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:  (a) New townhome construction, west exterior wall: On or about March 29, 2016, and at times prior, the employee exposed employees to collapse/fall hazards of approximately 20-feet, 10-inches from the pump jack scaffold platform, in that the supporting Alum-A-Poles were not resting on a firm foundation.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  (a) New townhome construction, west exterior wall: On or about March 29, 2016, and times prior, the employer exposed employees to fall hazards of 20-feet, 10-inches from the pump jack scaffold platform, in that personal fall arrest was not provided to employees while installing siding.
