29 CFR 1926.100(a):  Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns were not protected by protective helmets:  At a new residential construction site, employee walking and cutting material below workers was exposed to struck-by hazard of falling tools and materials and was not wearing a hard hat.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6-feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of '1926.502(k) developed and implemented.    At a new residential construction site, employees performing decking work on the roof without fall protection were exposed to fall hazards of more than 18-feet.
