29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling.  On or about 31 March 2016, and at times prior thereto, the employer allowed electrical equipment to be used in a manner not in accordance with the manufacturer's instructions included in the listing or labeling, exposing employees to an electrical hazard of approximately 120 volts. 1) Training Room: A re-locatable power tap was zip tied to the metal frame of the workstation rendering the power strip as permanent instead of temporary wiring. 2) Electrical/Lighting: A re-locatable power tap, extension cord, and three-way splitter were connected in series to provide power to a light fixture display at the end of the aisle, creating a daisy chain configuration.

29 CFR 1910.303(b)(7)(iv): 29 CFR 1910.303(b)(7)(iv): Electrical equipment had damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating.  On or about 31 March 2016, and at times prior thereto, the employer allowed damaged electrical energized parts to be used within the facility, exposing employees to an electrical hazard of approximately 120 volts. Receiving Bay 2 had an extendable dock light used for trailers in which the plug for the light and the light switch were separated from the metal frame of the arm. In this condition, the sheathed wires were visible but in close contact with sharp edges of the metal frame.

29 CFR 1910.305(a)(2)(ix): All lamps for general illumination were not protected from accidental contact or breakage by a suitable fixture or lamp holder with a guard.  On or about 31 March 2016, and at times prior thereto, overhead fluorescent lights located at various areas within the store were missing guards to prevent accidental contact or breakage, exposing employees to the hazards of cuts or blindness from falling shards of glass. 1) Aisle 14, Bays 6 through 10 had bulbs that were unprotected at a height of approximately 88 inches high (7 feet 4 inches) or less. 2) Aisle 15, Bay 19 had bulbs that were unprotected at a height of approximately 88 inches high (7 feet 4 inches) or less. 3) Aisle 59, Tool World, Bays 13 through 31 had bulbs that were unprotected at a height of approximately 88 inches high (7 feet 4 inches) or less.

29 CFR 1910.305(a)(2)(x):  Flexible cords and cables were not protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points.  On or about 31 March 2016, and at times prior thereto, flexible cords were not protected from accidental damage, exposing the employees working within the area to an electrical hazard of approximately 120 volts. 1) Within the Paint area, a workstation with wheels had been pushed up against a flexible cord that was plugged in at the time of the inspection, smashing the cord into an approximate 90 degree angle. 2) Within Outside Garden, Register 21, a power strip cord was setup so the cord ran through an open metal hole within the cashier's workstation.
