29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: a) SW Corner of the First Floor - On or about 03/17/2016, a carpenter who was installing floor joists was exposed to struck-by hazard and serious eye injury while operating a pneumatic nail gun out wearing eye protection. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) On or about 03/17/2016, carpenters performing wood framing were not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems while exposed to a 10 foot fall through a 42 inch by 7 foot unguarded door opening. b) SE Corner of First Floor -On or about 03/17/2016, carpenters performing wood framing were not protected from falling through an unguarded 3 foot by 16 foot opening by the use of guardrail systems, safety net systems, or personal fall arrest systems. c) First Floor, Ramp - On or about 03/17/2016, carpenters performing wood framing were not protected from falling 9 feet from a plywood ramp by the use of midrails, safety net systems, or personal fall arrest systems. d) SW Corner of First Floor - On or about 03/17/2016, carpenters performing wood framing were not protected from falling through an unguarded 8 foot by 8 foot opening by the use of guardrail systems, safety net systems, or personal fall arrest systems. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
