29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: Eagle River Ford service shop located at 1050 E. Wall St. Eagle River, WI: On and before March 31, 2016, employees were exposed to being struck by broken pieces of an abrasive wheel due to the tongue guard on the Sunex, model 5002A, 8" bench grinder not being adjusted perpendicularly to and within 1/4" of the wheel.

29 CFR 1910.134(a)(2): The employer did not establish and maintain a respiratory protection program which included the requirements outlined in 29 CFR 1910.134(c): Eagle River Ford body shop located at 1050 Wall St. Eagle River, WI: On and before March 31, 2016, employees were exposed to respiratory hazards in that procedures were not implemented to: 1) Inform the employees about the respiratory hazards in the workplace; 2) Train the employees to properly clean, maintain, and wear respirators; 3) Perform annual fit testing; 4) Perform medical monitoring.
