29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:       a) 2420 SR 20, Cazenovia, NY 13035, On or about 3/31/16: Employees were installing foot cleats on a metal roof that had a pitch of 9/12 and a fall distance of 11 feet from the eaves edge to the ground elevation below. without using proper fall protection equipment.    b) 2420 SR 20, Cazenovia, NY 13035, On or about 3/30/16: Employees were installing metal roofing on a roof that had a pitch of 9/12 with a fall distance of 11 feet from the roofs eave to ground elevation below without using proper fall protection.
