29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: a) Worksite at 25 Chapel Street, Albany, NY - On or about March 31, 2016, an employee of a subcontractor was working inside an aerial lift without fall protection. Employees were installing exterior insulation and finishing system for the roofing system and the lift was used at the top of the building which was 111' to the ground.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a) Worksite at 25 Chapel Street, Albany, NY - On or about March 31, 2016 an employee of a subcontractor was exposed to a fall hazard of approximately 111' from the roof of a hotel to the ground while standing on the parapet wall to install the exterior insulation and finishing system. The employee was observed conducting this work without fall protection and both hands overhead.
