Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  a)  Worksite at 25 Chapel Street, Albany, NY - On or about March 31, 2016 an employee of a subcontractor was exposed to a fall hazard of approximately 111' from the roof of a hotel to the ground while standing on the parapet wall to install the exterior insulation and finishing system.  The employee was observed conducting this work without fall protection and both hands overhead.
