29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.      A. Just 1 Crew worksite located on Coffee Tree Lane (Carlisle Commons), Sylvania, Ohio: On or about March 29, 2016, the employer did not ensure employees were wearing proper eye protection while operating pneumatic nail guns to secure asphalt shingles on the single story residential structure. The employees were exposed to potential flying object hazards while operating the pneumatic fasteners.

29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.        A. Just 1 Crew worksite located on Coffee Tree Lane (Carlisle Commons), Sylvania, Ohio: On or about March 29, 2016, the employer did not ensure employees were protected from head injuries while working below employees performing weatherproofing activities. Due to this condition, employees were potentially exposed to impact injuries caused from falling material and/or equipment.

A. Just 1 Crew worksite located on Coffee Tree Lane (Carlisle Commons), Sylvania, Ohio: On or about March 29, 2016, the employer did not ensure employees were protected from falls while performing weatherproofing activities on the single story residential structure. Employees were exposed to a fall distance of approximately 8.00 feet from the 6/12 pitch roof.
