29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stair rail system along each unprotected side or edge:    At the jobsite, the stairs leading to the second floor were not equipped with handrails, exposing employees to the hazard of falling.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.   At the jobsite, employees operating Hitachi nail guns while performing roofing activities were not wearing eye protection, exposing employees to the hazard of being struck by flying objects.   Juan Guadalupe was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1133570, Citation Number 1, Item number 1, and was affirmed as a final order on May 5, 2016, with a final abatement date of May 22, 2016, with respect to a workplace located at 1707 Legacy Lane, Waco, TX 76712.

At the jobsite, employees engaged in roofing activities at a height of over 6 feet were not protected by a fall arrest system. Employees were exposed to an unprotected fall of over 20 feet to the ground below.   Juan Guadalupe Sanchez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1133570, Citation Number 1, Item number 2, and was affirmed as a final order on May 05, 2016, with a final abatement date of May 2, 2016, with respect to a workplace located at 1707 Legacy Lane, Waco, TX 76657
