29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection.  Location:  Eaton Door section of the building On or about 4/1/16, Employees were using portable paint sprayers receiving power from extension cords that were not equipped with a GFCI.

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening is 6 feet or more above lower levels and the inside bottom edge of the wall opening  Location: Eaton Door section of the building On or about 4/1/16 an employee was not protected from falling while working on an elevated surface more than 6 feet above a lower level.
