29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  At 2512 West Beach Blvd. Gulf Shores, Alabama:  On or about March 29, 2016 and at times prior; the employer allowed employees to walk  the top plate of the third floor wall without fall protection exposing the employee to a fall hazard of 10 feet to the floor below.  b)  At 2512 West Beach Blvd. Gulf Shores, Alabama:  On or about March 29, 2016 and at times prior; the employer allowed employees to work from the top plate of the third floor rear wall without fall protection exposing the employee to a fall hazard of 20 feet to the floor below.   c)  At 2512 West Beach Blvd. Gulf Shores, Alabama:  On or about March 29, 2016 and at times prior; the employer allowed employees to work from the first floor wall opening without fall protection exposing the employee to a fall hazard of 10 feet to the floor below.  d)  At 2512 West Beach Blvd. Gulf Shores, Alabama:  On or about March 29, 2016 and at times prior; the employer allowed employees to work from the second floor wall opening without fall protection exposing the employee to a fall hazard of 20 feet to the floor below.  e)  At 2512 West Beach Blvd. Gulf Shores, Alabama:  On or about March 29, 2016 and at times prior; the employer allowed employees to work from the third north west side without fall protection exposing the employee to a fall hazard of 30 feet to the floor below.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step:  a) At 2512 West Beach Blvd. Gulf Shores, Alabama:  On or about March 29, 2016 and at times prior; the employer allowed employees to use the top step of an a-frame ladder to gain access to the second level of the house exposing the employee to a fall hazard of 10 feet to floor below.  b) At 2512 West Beach Blvd. Gulf Shores, Alabama:  On or about March 29, 2016 and at times prior; the employer allowed employees to use the top step of an a-frame ladder to gain access to the third level of the house exposing the employee to a fall hazard of 10 feet to floor below.
