29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  a) Greenburg Building - The employee was exposed to an electrical hazard from two (2) extension cords not having a continuous ground, on or about March 29, 2016.    b) Greenburg Building - The employee was exposed to an electrical hazard from a Bosch radio not having a continuous ground, on or about March 29, 2016.    c) Greenburg Building - The employee was exposed to an electrical hazard a Patton fan not having a continuous ground, on or about March 29, 2016.      Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  a) Greenburg Building - The employer did not ensure electrical equipment was free from recognized hazards exposing the employee to electrical hazards from two (2) extension cords, a fan and a radio missing the grounding pin, on or about March 29, 2016.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
