29 CFR 1910.134(c)(2)(i): Where the employer determined that any voluntary respirator use is permissible, the employer did not provide the respirator users with the information contained in Appendix D to this section ("Information for Employees Using Respirators When Not Required Under the Standard").  On or about April 05, 2016 the employer provided respirators for voluntary use including "3M" 8511 filtering facepieces, "3M" model 5200 and 7586 tight fitting 1/2 face respirators but did not also provide the affected employees with the information in Appendix D of 1910.134.

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written respiratory protection program necessary to ensure that any employee using a respirator voluntarily is medically able to use that respirator, and that the respirator is cleaned, stored, and maintained so that its use does not present a health hazard to the user.  On or about April 05, 2016 the employer provided "3M" 1/2 mask, tight fitting respirators for voluntary use but did not develop a written respiratory protection program or provide medical evaluations to affected employees.

The employer did not provide a guard adequate to protect employees from amputation hazards from the blade of the saw.
