29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition:   On 05/05/2016, employees working at SuperVan Service Co., Inc. located at 121 Bremen Ave., St. Louis, MO 63147, were exposed to a forklift in need of repair.  The locator/index pin was missing on the 30 Nissan forklift.

29 CFR 1904.32(b)(3): How do I certify the annual summary? A company executive must certify that he or she has examined the OSHA 300 Log and that he or she reasonably believes, based on his or her knowledge of the process by which the information was recorded, that the annual summary is correct and complete:   The company executive for SuperVan Service Co., Inc. located at 121 Bremen Ave., St. Louis, MO, did not certify the OSHA 300-A form for 2015 (nor the previous years of 2014 and 2013).  The HR Manager signed the forms instead of the Director of Operations.
