29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  a) On or about April 4, 2016, an employee using a 7-1/4" potable circular saw to cut roof sheathing to size and was not protected from eye injuries caused by dust and flying debris because he was not protected with proper eye protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a)  On or about April 4, 2016 4 employees installing roof sheathing on a 5:12 pitch low slope, one story residential structure and were exposed to falls in excess of 10 feet to the ground below, because they were not protected by any means of Personal Fall Protection, Guard rails, or Safety nets.
