29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     (a)  Jobsite, 3959 Greensburg Pike, Pittsburgh, PA:  On or about 04/05/16 employees were working on the roof of a structure approximately 12-14 feet above the ground and no guardrail systems, safety net systems or personal fall arrest systems were in place to prevent the employees from falling.

29 CFR 1926.503(c)(3): Retraining had not been conducted when inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill.  Note: The following appendices to subpart M of this part serve as non-mandatory guidelines to assist employers in complying with the appropriate requirements of subpart M of this part.[59 FR 40738, Aug. 9, 1994; 60 FR 5131, Jan. 26, 1995]:  (a) Facility, 3959 Greensburg Pike, Pittsburgh, PA:  On or about 04/05/16 employees had not been retrained on fall protection when inadequacies in the employee's knowledge or use of fall protection systems indicated that they had not retained the skills and understanding to ensure they protected themselves from fall hazards.  Employees were working on levels approximately 12-14 feet above the ground and no fall protection systems were in place indicating employees had not retained the knowledge and skill to protect them when exposed to fall hazards.
