29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):     (a) Burrell Hume & Associates, Inc. dba Enviro-Tech Roofing at 9755 East Hampden Avenue, Denver, CO 80231:  On and before April 8th, 2016 the employer did not ensure that frequent and regular inspections by a competent person were conducted to identify work site hazards.  Employees were performing roofing operations without fall protection on a two-story roof with a steep roof.  This condition exposed employees to fall hazards from 12 to 20 feet pending upon location on the roof.
