29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours:  a.   On or about April 6, 2016, on the F1 Basket Elevator Hoist, an employee suffered an injury to his left hand, causing the employee to be hospitalized, and the employer did not report the work-related accident to OSHA within 24 hours.

29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:  a.  On or about April 6, 2016, the energy control procedures for the F1 Basket Elevator Hoist did not account for the potential energy stored in the hoist, exposing an employee to a crush hazard.

On or about April 6, 2016, at the F1 Basket Elevator Hoist, an employee working as an authorized employee was not provided the equipment necessary to guard against all sources of energy, exposing the employee to a crush hazard.

On or about April 6, 2016, at the F1 Basket Elevator Hoist, an employee trained as an affected employee was working as an authorized employee, but was not provided authorized employee training, exposing the employee to a crush hazard.
