In the warehouse at 345 Central Street, Leominster, MA 01453, on 4/13/2016 and at times earlier 3 of 3 powered industrial truck (PIT) operators ran PITs without having received initial training in workplace-related topics  such as, but not limited to: surface conditions where the vehicle will be operated; composition of loads to be carried and load stability; load manipulation, stacking, and unstacking; pedestrian traffic in areas where the vehicle will be operated; narrow aisles and other restricted places where the vehicle will be operated.
