Test voltages and the maximum intervals between tests shall be in accordance with Table I-4 and Table I-5.    a. St. Rita's Health Partners located in Lima, Ohio:  On or about April 12, 2016, the employer did not ensure insulated gloves available for use by the electricians, were tested at six month intervals. Class I gloves #150-1200, assigned for use to an electrician, were last tested on September 1, 2015, and were due for inspection on March 1, 2016.

29 CFR 1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which are or may be energized. The specific safety-related work practices were not consistent with the nature and extent of the associated electrical hazards.    a. St. Rita's Health Partners located in Lima, Ohio:  On or about March 8, 2016, the employer did not ensure electrical safety-related work practices were used when an electrician was performing work on a lighting system located on the sixth floor of the hospital in the renal telemetry department.  The electrician disconnected the energized conductors from the light ballast without first activating the breaker that was supplying power to the conductors.    b. St. Rita's Health Partners located in Lima, Ohio:  On or about March 8, 2016, the employer did not ensure electrical safety-related work practices were used when an electrician was performing work on a lighting system located on the sixth floor of the hospital in the renal telemetry department.  A second electrician disconnected energized conductors housed in a junction box which supplied power to the lighting circuit without first activating the breaker that was supplying power to the conductors.

29 CFR 1910.333(b)(2)(ii)(B): The circuits and equipment to be worked on was not disconnected from all electric energy sources. Control circuit devices, such as push buttons, selector switches, and interlocks, were used as the sole means for deenergizing circuits or equipment. Interlocks for electric equipment were used as a substitute for lockout and tagging procedures.    a. St. Rita's Health Partners located in Lima, Ohio:  On or about March 8, 2016, the employer did not ensure an electrician disconnected electrical power from a lighting system located on the sixth floor of the hospital in the renal telemetry department, prior to performing work on the equipment. The electrician disconnected the energized conductors supplying power to the light ballast without first activating the breaker that was supplying power to the conductors.    b. St. Rita's Health Partners located in Lima, Ohio:  On or about March 8, 2016, the employer did not ensure an electrician disconnected electrical power from a lighting system located on the sixth floor of the hospital in the renal telemetry department, prior to performing work on the equipment.  The second electrician disconnected energized conductors housed in a junction box which supplied power to the lighting circuit without first activating the breaker that was supplying power to the conductors.

29 CFR 1910.334(b)(1):  Load rated switches, circuit breakers, or other devices specifically designed as disconnecting means were not used for the opening, reversing, or closing of circuits under load conditions. Cable connectors not of the load break type, fuses, terminal lugs, and cable splice connections may not be used for such purposes, except in an emergency.    a. St. Rita's Health Partners located in Lima, Ohio:  On or about March 8, 2016, the employer did not ensure the circuit breaker for the night lights located on the sixth floor of the hospital in the renal telemetry department, was opened to deenergize the lighting circuit.  An employee disconnected the energized conductors to the light ballast by removing the wire nuts on the energized conductors as electrical power was being supplied to the light.    b. St. Rita's Health Partners located in Lima, Ohio:  On or about March 8, 2016, the employer did not ensure the circuit breaker for the night lights located on the sixth floor of the hospital in the renal telemetry department, was opened to deenergize the lighting circuit.  An employee disconnected the energized conductors to the lighting circuit by removing the wire nuts on the conductors located in a junction box and then separating the conductors supplying power to the light.
