OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: employees were operating manually propelled elevating aerial platforms without the proper use of outriggers.      (a)  On or about April 12, 2016, the outriggers for the Handy Herman Model 24D aerial platform were not properly extended while the platform was being used by an employee to access the motor of a gymnasiums mechanically driven dividing curtain. This exposed the employee to a tipping/fall hazard of approximately 25 feet to the gymnasium floor.    Among other methods, feasible and acceptable means to correct the hazards are to use a safer alternative method such as:    1. Follow the operator's manual, which was located on the aerial platform, and states on pages 4 and 5:  Manual Outriggers;  Outriggers must be extended whenever the platform is elevated.  When positioning the unit for work, leave approximately 25 inches of space for outrigger extension between the unit and any obstruction on both sides on the machine (Page 4) and All Models  Always extend and adjust all outriggers so base is level before extending (Page 5).      2.   Follow the Association of Equipment Manufacturers Aerial Platform Safety Manual for Operating and Maintenance Personnel. On page 31 it states Raise and Lower Safely  Make sure machine is on firm and level ground before raising the platform.  If so equipped, make sure extendible axles, outriggers, or stabilizers are fully deployed.  Outriggers or stabilizers may require blocking to provide a stable load bearing surface.  Always check clearance on both sides of the machine before extending outriggers, stabilizers or axles.      3.   Follow requirements outlined in the American National Standards Institute (ANSI)  ANSI/SIA A92.3-1990 American National Standard for Manually Propelled Elevating Aerial Platforms - Section 7.9(2) During Operation. That the outriggers, stabilizers, extendable axle, or other stabilizing methods are used as required by the manufacturer.
