29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agent:  Employees engaged in framing activities using Hitachi NR83A3 3.25" Pneumatic Framing Strip Nailer at 3130 N Parkdale Circle, Wichita, KS 67205 located at the west wing, north side of the construction site were exposed to struck by hazards to the eyes in that eye protection was not worn.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system or personal fall arrest systems:  Employees engaged in framing activities at 3130 N Parkdale Circle, Wichita, KS 67205 located at the west wing, north side of the construction site were exposed to fall hazards of 11 feet from the balcony to the ground in that fall protection was not used.
