29 CFR 1910.305(g)(2)(iii) as required by 1960.8(b): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: VA Traverse City CBOC- In the lab, adequate strain relief on the cord of an electrical surge protector was not provided thereby exposing employees to a shock hazard.

29 CFR 1910.157(g)(3) as required by 1960.8(b): Employees who have been designated to use fire fighting equipment as part of the emergency action plan were not provided training in the use of the appropriate equipment: VA Traverse City CBOC, the employer did not provide annual training on portable fire extinguishers that included hands-on training to employees designated to use portable fire extinguishers. The Veterans Health Administration was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.157(g)(3), which was contained in OSHA inspection number 896779, citation number 1, item number 1 and was affirmed as a final order on April 26, 2013, with respect to a workplace located at 806 Otsego Rd., Gaylord, Michigan 49735.
