29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Jobsite, 7669 Lincoln Highway, Bedford, PA:  On or about April 14, 2016, an employee was installing siding on a residential roof and was not protected from falling.  The fall hazard to the employee was approximately 14 feet to the ground.  b) Jobsite, 7669 Lincoln Highway, Bedford, PA:  On or about April 14, 2016, an employee was installing siding on a residential roof, standing at the top of a 4 foot Werner portable step ladder, and was not protected from falling.  The fall hazard to the employee was approximately 18 feet to the ground.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards:  a) Jobsite, 7669 Lincoln Highway, Bedford, PA:  On or about April 14, 2016, 3 employees were working on a residential roof at heights above 6 feet and were not protected from falling.  The employer was not able to provide any documentation of fall protection training provided to the employees.
