29 CFR 1926.451(b)(3): Where guardrails were not installed along the front edge of the platform or all employees were not protected from falling with the use of personal fall arrest systems, the front edge of the scaffold platform was more than 14 inches:    (a) Rear of house:  Employees were working approximately fifteen (15) feet above the ground on ladder-jack scaffolding where the front edge was approximately twenty-four (24) inches from the edge of the building without the use of fall protection exposing employees to a fall to the concrete below.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person.  (a) Rear of house:  A ladder-jack scaffold system was not erected under the supervision of a competent person.  This allowed the scaffolding to be erected improperly exposing employees to a fall of approximately fifteen (15) feet to the concrete walkway below and struck-by hazards from falling scaffold components.
