29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    On or about April 19, 2016, an employee was not protected from a fall while walking/ working from the roof of a mobile trailer, exposing the employee to a 14 foot fall.    Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet.     On or about April 19, 2016, an employee used an extension ladder that did not extend three feet past the upper landing, exposing the employee to a fall of 14 feet.     Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
