29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):    a). On or about 4/8/16, on site, for employees using chemicals for commercial screening. The employee use an automatic screen printing machine. The employee uses Ryonet Mist Adhesive to adhere each shirt to the surface of the printing machine. The Ryonet Mist Adhesive contains chemicals, such as, but not limited to, Butane and Cyclohexane. The employer did not conduct a hazard assessment.    b). On or about 4/8/16, on site, foe employees using chemicals for commercial screening. The employee use an automatic screen printing machine. The employee uses Ryonet Screen Clean to clean the the screens on the printing machine. The Ryonet Screen Clean contains chemicals, such as, bit not limited to, Cyclohexanone. The employer did not conduct a hazard assessment.

On or about 7/15/16, on site, for an employee using a pressure washer that is plugged to an extension cord. The extension cord is plugged into a Relocatable Power Taps (RPTs). The extension cord also powers a water heater and another extension cord which provides power to another RPTs, a dehumidifier, and a fan.
