29 CFR 1910.27(d)(1)(v): Vertical bars on the fixed ladder safety cage were not located at a maximum spacing of 40 degrees around the circumference of the cage giving a maximum spacing of approximately 9 1/2 inches, center to center:    a) Sand Silo #1: Vertical safety bars were missing between the second and third horizontal rings exposing employees to a potential 15-20 foot fall hazard, as observed on or about April 20, 2016.    Abatement Certification Required.

29 CFR 1910.27(b)(2): Side rails that could be used as a climbing aid were not of such cross sections as to afford adequate gripping surface without sharp edges, splinters, or burrs:  a) Sand silo #1: Electrical conduit was attached to the left side rail thus not affording an adequate grip and sharp edges were created where side rails were spliced together, as observed on or about April 20, 2016.  Abatement Certification Required.
