29 CFR 1910.27(d)(1)(v): Vertical bars on the fixed ladder safety cage were not located at a maximum spacing of 40 degrees around the circumference of the cage giving a maximum spacing of approximately 9 1/2 inches, center to center: a) Sand Silo #1: Vertical safety bars were missing between the second and third horizontal rings exposing employees to a potential 15-20 foot fall hazard, as observed on or about April 20, 2016. Abatement Certification Required.
