This violation was observed on or about April 20, 2016 at 4913 W. Reno, Oklahoma City, Central Oklahoma American Indian Health Council, Inc. did not ensure that each employee was wearing a safety harness with lanyard attached to the proper anchor points while in the JLG Model T350 trailer mounted boom aerial lift exposing employees to a fall from approximately 25 ft. to the ground below.
