29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  a). On or about 4/21/16, on site, when dispensing Blaser Swisslube coolant, a hazard assessment was not conducted  to require the use of nitrile gloves when dispensing the coolant into the CNC machines. The employer provides safety gloves. The Blaser Swisslube Blasocut 4000 Strong coolant contains chemicals such as, but not limited to, Emulsifiers and Mineral Oil.

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2):  a). On or about 4/21/16, on site, when dispensing Blaser Swisslube coolant, a hazard assessment was not conducted  to require the use of nitrile gloves when dispensing the coolant into the CNC machines. The Blaser Swisslube Blasocut 4000 Strong coolant contains chemicals such as, but not limited to, Emulsifiers and Mineral Oil.
