On or about April 20, 2016, at the above addressed jobsite, employees were performing bricklaying operations from a tubular welded frame scaffold located north on the west side of the building. The scaffold had a height to base width ratio of more than four to one (4:1) and was not secured with guys, ties and/or braces, thereby exposing employees to fall hazards. b. On or about April 20, 2016, at the above addressed jobsite, an employee was performing bricklaying operations from a tubular welded frame scaffold located south on the west side of the building. 29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access. a. On or about April 20, 2016, at the above addressed jobsite, employees were performing bricklaying operations from a tubular welded frame scaffold located north on the west side of the building approximately twenty (20) feet above the ground. The employees were not provided with a ladder access, thereby exposing the employees to fall hazards. b. On or about April 20, 2016, at the above addressed jobsite, an employee was performing bricklaying operations from a tubular welded frame scaffold located south on the west side of the building approximately twenty (20) feet above the ground. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. a. On or about April 20, 2016, at the above addressed jobsite, employees were performing bricklaying operations from a tubular welded frame scaffold located north on the west side of the building approximately twenty (20) feet above the ground. The employees were not provided with a guardrail system or personal fall arrest system, thereby exposing the employees to fall hazards. b.
