29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) At the leasing office, on March 28, 2016, the employer did not ensure employees who were performing painting work on the chimney of the office building were equipped with harnesses, or other means of fall protection, during the course of their work, exposing employees to a 12-foot fall hazard.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  a) At the leasing office, on March 28, 2016, an employee placed an unsecured 10-foot aluminum extension ladder on the surface of a steep-pitch roof to access the chimney of the office to paint and thus was exposed to fall hazards.
