29 CFR 1926.1060(a)(1): The employer did not provide a training program for each employee using ladders as necessary, enabling each employee to recognize hazards related to ladders and the procedures to be followed to minimize these hazards: On or about April 21, 2016, employees using a ladder to install sprinkler heads for fire protection had not been trained to recognize hazards related to ladders and the procedures to follow that would minimize the hazards.
