29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Diversified Design & Manufacturing, Inc. - On or about April 20, 2016, a Peck Stow & Wilcox brand mechanical shearing machine was not guarded by physical barriers, physical devices, or by restricting access to prevent employees from having their fingers inadvertently in the danger zone of the shear blades during operating cycles. b) Diversified Design & Manufacturing, Inc. - On or about April 20, 2016, a DoAll brand vertical contour band saw was being used without a guard to protect employee's hands from inadvertently contacting the moving blade. c) Diversified Design & Manufacturing, Inc. - On or about April 20, 2016, the interlock device for the movable door guard of the Wasino brand LG-81 Gang Lathe was defeated with a plastic tie exposing employees to rotating parts, and or flying metal chips. d) Diversified Design & Manufacturing, Inc. - On or about April 20, 2016, the interlock device for the movable door guard of the Wasino brand LJ-6 Turning Center was defeated with a plastic tie exposing employees to rotating parts, and or flying metal chips. ABATEMENT NOTE: Among other methods, one feasible and acceptable abatement method to correct these hazards is to guard the point of operation hazards in accordance with American National Standard ANSI B11.4-2003, safety Requirements for Shears, and ANSI B11.22-2002, Safety Requirements for Turning Centers and Automatic, Numerically Controlled Turning Machines.
