29 CFR 1926.451(c)(1)(ii): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by installing guys, ties, and braces according to the manufacturer's recommendations:   (a) East side, new residential construction located at 828 Adler Court, Alpharetta, GA 30009: Employees were exposed to collapse /fall hazards while installing bricks from a tubular weldd scaffold system which was 5 sections high and not secured to the structure to prevent tipping or collapse.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  (a) East side, new residential construction located at 828 Adler Court, Alpharetta, GA 30009: An employee was exposed to falls of approximately 30-feet, 6-inches while working from a tubular welded scaffold platform without a guardrail system or other fall protection system in place.
