29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe-boards, safety net systems, or personal fall arrest systems: a) On or about 04/25/2016 - Lot 6, Highland View, Lincoln, AL, employees installing shingles on the 7:12 sloped roof were not protected from falls of approximately 12 feet. b) On or about 04/21/2016 - Lot 7, Highland View, Lincoln, AL, employees installing shingles on the 10:12 sloped roof were not protected from falls of approximately 12 feet.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder: a) On or about 04/25/2016 - Lot 6, Highland View, Lincoln, AL, employees were using a Werner aluminum extension ladder to gain access to the roof that did not extend 3 feet or more above the landing.
