29 CFR 1926.502(i)(2): Cover was not capable of supporting, without failure, at least twice the weight of the employees, equipment, and materials that maybe be imposed on the cover at any one time:       a)  Alley, Areaway - On or about 03/16/2016, a worker standing on  a  3/4 inch OSB plywood  hole cover fell 30 feet down through an areaway shaft when cover collapsed.      No abatement certification or documentation required for this item.
